Our Mission
We help people stay stable and healthy at home, with care that never loses sight of them. A serious health condition means navigating a system that only sees patients in appointments. Our clinicians cover the time in between, keeping patients stable, more independent, and out of the hospital.
Our Story
In 2002, Nesim Bildirici lost his father, Gabriel, to heart failure and to a system that only saw him during appointments. He believed his father would have lived longer if his care team could have stayed with him between visits. Nesim founded AMC Health with the belief that no patient should be out of sight in between visits. Twenty-four years later, that belief now operates nationally, providing continuous care for patients across the country.

Fewer heart failure admissions, lower cost of care, 3.3x return
Geisinger Health Plan added AMC Health home monitoring to the case management it already ran for Medicare Advantage members with heart failure. Over five years of claims data, enrolled members were admitted less often, readmitted less often, and cost the plan 11% less. Published in Population Health Management.
