Continuous care, no matter the distance
Distance and staffing shortages leave rural patients unseen between visits. Keeping them stable at home takes daily readings, clinicians who act on them, and a team that extends yours.
Clinical Capacity
Nurses and care managers who monitor readings, act on alerts, and close every order, referral, and test. Staffed by our team or yours, with your physician’s care plan governing.
Clinically- Governed AI
Models trained on two decades of outcomes cut through alert noise and direct attention to the patient whose risk is forming. They inform clinical judgment, never replace it.
Daily Data From Every Home
Cellular devices and phone check-ins work without home internet or a smartphone. Readings reach your clinicians where they already work, so distance never delays a decision.
Built to outlast the Rural Health Transformation Program
RHTP dollars run through 2030. Whether you’re launching an award, planning an application, or already running RPM, the program is built to sustain itself on CMS reimbursement.
An enrollment playbook, devices, and clinical staffing from day one, so patients are monitored before your first reporting deadline.
Our nurses and care managers monitor, triage, and follow up. Your clinicians keep clinical authority without adding headcount.
Twenty years of large-scale programs, including the VA’s. We bring the workflows, training, and integration so your team isn’t building from scratch.
Outcomes reporting aligned to the measures your state tracks: utilization, chronic condition control, and cost.
Programs built on CMS reimbursement for remote monitoring and care management fund themselves. Billing setup is part of implementation, not an afterthought.
Keep patients
close to home
Every transfer out and every post-discharge gap sends patients and revenue away from your hospital. Daily post-discharge monitoring catches deterioration early and keeps patients from being readmitted.
Two decades serving rural veterans
The VA runs the country’s largest remote monitoring program, and many of its veterans live in rural communities. AMC Health has been a vendor on the VA’s national Home Telehealth contract, supplying the logistics, connectivity, and clinical reach that rural care demands.
PEER-REVIEWED STUDY
Fewer heart failure admissions, lower cost of care, 3.3x return
Geisinger Health Plan, which serves a largely rural population in central Pennsylvania, added AMC Health home monitoring to the case management it already ran for Medicare Advantage members with heart failure. Over five years of claims data, enrolled members were admitted less often, readmitted less often, and cost the plan 11% less. Published in Population Health Management.

Common rural health questions
What the program covers, how it fits your community, and what it takes to run.
Which conditions does the program cover?
Hypertension, diabetes, heart failure, and COPD, with patients managing several at once the norm. Post-discharge monitoring covers any patient at risk of readmission, whatever brought them in.
How does it work with EMS and paramedicine?
Alerts that need eyes in the home can route to community paramedics for in-person assessment, and paramedics can set up devices on existing visits. State RHTP plans fund both models, and they work better together.
What if patients don’t have internet?
Cellular devices and phone-based check-ins work without broadband or a smartphone. Patients who have never used an app can still be monitored every day.
Can it support behavioral health?
Daily check-ins can include depression and anxiety screening, with results routed to your care team. It extends behavioral health access in communities short on providers. It does not replace them.
Does it integrate with our EHR?
Readings and clinical documentation flow into your EHR so your clinicians work where they already work. Integration is part of implementation, not a separate project.
Can our own nurses run the program?
Yes. Your staff can run it on our platform, our clinicians can run it end to end, or you can start with our team and shift as capacity grows.
