A program your employees will use
A benefit only pays for itself when people use it. We identify eligible employees and handle outreach and setup ourselves, so nothing lands on your benefits team. Once someone starts, a nurse follows their readings, and if they stop, the nurse calls to find out why.
Fits inside the plan you have
Adding a program shouldn’t mean changing your carrier or your plan design. We work within your current medical plan and fit around the programs you already run, including an on-site clinic. Employees keep the coverage they know, and you add daily clinical attention for the ones who need it.
RANDOMIZED TRIAL
Blood pressure controlled from home, results that held, in a randomized trial
HealthPartners, an integrated health system in Minnesota, gave adults with uncontrolled hypertension, half of them still working, AMC Health home blood pressure monitors and had clinical pharmacists adjust their medications from the readings by phone. Within six months most patients had their blood pressure under control, compared with fewer than half in usual care, and the gap was still there six months after the program ended. Published in JAMA.
