GOVERNMENT
Care that reaches everyone you’re responsible for
The people public programs serve often live far from a clinic, without broadband, or inside a facility, and their conditions deteriorate between appointments. AMC Health’s clinicians cover those days, wherever the patient is.

Continuous care for your hardest-to-reach populations
Chronic conditions change daily, and most of those changes happen between appointments. A problem that goes unseen for weeks usually arrives as an admission or a referral outside your system.
Every Patient Can Join
Devices report by app, cellular tablet, modem, or a plain phone call, and our team handles enrollment and setup, so connectivity and comfort with technology never decide who takes part.
Adds Clinical Capacity
A licensed, multidisciplinary team reviews each day’s readings and adjusts treatment under the care plans your clinicians set, so daily coverage holds where clinicians are scarce.
Outcomes You Can Report
Every encounter is documented with a full audit trail and reported against the measures your program is held to, so the effect on admissions and referrals is ready when oversight asks.
Proven at the scale of the VA
The VA runs the largest remote patient monitoring program in the country. AMC Health’s platform and clinical team deliver one of the four awards on its national Home Telehealth contract, and have served the program across two award cycles.
AMC Health built and operates the FDA Class II–cleared platform VA care coordinators use to monitor Veterans with complex chronic conditions at home. Daily readings, risk flags, and the clinical view that turns them into action, nationwide.
Kits ship to the Veteran’s door and connect by cellular hub, app, or a plain phone call, so Veterans without home internet take part. Setup, training, replacements, and support run nationally.
Zero Trust–aligned controls, role-based access, encryption in transit and at rest, and Section 508–ready experiences. Standards-based interoperability (FHIR/HL7) supports ATO and audit requirements and connects to VistA and Oracle Health.
Fewer escalations, steadier budgets
The costly part of chronic disease is the escalation, the admission or the care purchased outside the system after a condition went unmanaged for weeks. Daily monitoring and treatment keep the patients most likely to escalate stable, so the funds set aside for outside care last the year and go to the cases that need them.
From one pilot to a full program
Most programs begin with one population at one site, often a cohort with uncontrolled hypertension or diabetes. We bring the devices, logistics, clinicians, and reporting from the first enrollment, so a pilot starts without new hires or new systems and can grow across a region once the results are in.
OBSERVATIONAL STUDY
Fewer heart failure readmissions, lower cost of care, for a national Medicaid plan
A national managed Medicaid plan added AMC Health home monitoring and nurse care management for members with Stage II+ heart failure in four states. Against propensity-matched members who weren’t enrolled, program members were readmitted less often and cost the plan 20% less over the program year. Retrospective analysis of claims data, 2021–22.
