HEALTH PLANS
Your highest-risk members no longer need to be your most expensive
Most plans already know which members will drive next year’s cost. The deterioration behind that cost starts between appointments, long before it reaches a claim. AMC Health’s clinicians cover those days.

Continuous care between visits
Keeping high-risk members stable at home requires clinicians with the authority to treat, models that guide them to the right member, and daily data from the home.
Licensed Clinicians
A multidisciplinary team of clinicians who prescribe and adjust treatment, with care managers who follow every order, referral, and test through to completion.
Clinically- Governed AI
Models trained on two decades of outcomes cut through alert noise and direct attention to the member whose risk is forming. They inform clinical judgment, never replace it.
Daily Data From the Home
Each day’s readings, combined with the claims, pharmacy, and EHR data your plan shares, build a longitudinal record of how and when risk develops for every member.
You Choose the Level of Accountability We Hold
We partner with plans in three ways, and each carries more clinical accountability. We can equip your care team with daily data from the home, extend a member’s physician's care with our care managers and clinicians, or serve as the member’s primary care provider.

Care that reaches every member
Many members live where broadband is unreliable, prefer a phone to an app, or have never joined a program. We handle outreach, consent, and setup, and connect each member through whatever fits their home, from a smartphone app to a cellular tablet to a plain phone line. Where a member lives never decides whether they can take part.
Fewer admissions, stronger measures
Most quality measures are decided between visits by whether blood pressure stays controlled and HbA1c comes down. Daily care at home moves those measures for the members who most often miss them, and your plan gets the credit. Admissions fall for the same members, so cost and quality improve together.
Built for every line of business
Daily clinical attention for the members who carry your Star measures and medical cost, keeping them stable at home between visits.
Reaches members wherever they live, with connectivity that needs no broadband or smartphone, and keeps quality measures moving across a changing
Continuous care for members living with several chronic conditions, delivered at home by clinicians who act before a crisis forms.
Chronic condition care fitted around working lives, with outcomes you can bring to employer groups and exchange members.
OBSERVATIONAL STUDY
Fewer heart failure readmissions, lower cost of care, for a national Medicaid plan
A national managed Medicaid plan added AMC Health home monitoring and nurse care management for members with Stage II+ heart failure in four states. Against propensity-matched members who weren’t enrolled, program members were readmitted less often and cost the plan 20% less over the program year. Retrospective analysis of claims data, 2021–22.
