PROVIDERS
Care that continues when your patients go home
Patients with chronic conditions spend a few hours a year with your care team and every day at home, where deterioration starts. AMC Health’s clinicians cover those days, alongside your physicians and within the workflows you already run.

Continuous care, alongside your team
High-risk patients need attention every day, more than any schedule can hold. Our clinicians take it on, so your team’s time goes to the patients who need them in person.
Led by Your Physicians
Clinicians who prescribe and adjust treatment under your physician’s care plan, with care managers who follow every order, referral, and test through to completion.
Time Back for Your Team
Our clinicians review every reading, act on every alert, and handle the outreach and follow-up in between, guided by models trained on two decades of outcomes.
Documented in Your EHR
Daily readings, our clinicians’ notes, and every order and referral are written back to your EHR via bidirectional integration, so your physicians work from a single record.
You Choose the Level of Accountability We Hold
We partner with providers in three ways, and each carries more clinical accountability. We can equip your care team with daily data from the home, extend your physicians with our care managers and clinicians, or become the primary care provider for patients who don’t have one, referring in-person care to your network.

Care that reaches every patient
Many patients go home to places with unreliable broadband, prefer a phone to an app, or have never used a connected device. From your referral onward, we handle outreach, consent, and setup, connecting each patient through whatever fits their home. Where a patient lives and how comfortable they are with technology never decides whether they can get connected.
Fewer readmissions, safer discharges
The first weeks after discharge determine whether a patient recovers at home or returns to the emergency department. From the first days home, our clinicians review each day’s readings, act on early signs of decline, and keep your physicians informed through the record. Readmissions fall among the patients most likely to return, and the beds they would have filled go to patients who need them.
Built for every provider
Readmissions fall for heart failure, COPD, and the other penalized conditions, and patients discharged without a primary care physician go home with one.
Daily care for the attributed patients who drive your total cost of care, with the blood pressure and A1c control that count toward your quality score.
Our clinicians carry the daily monitoring and follow-up, so chronic care grows without new hires, and Medicare reimburses the program monthly, per patient.
Patients are connected without broadband or a smartphone, and the hypertension and diabetes measures in your UDS report improve for the patients hardest to reach.
PEER-REVIEWED STUDY
Fewer heart failure admissions, lower cost of care, 3.3x return
Geisinger Health Plan added AMC Health home monitoring to the case management it already ran for Medicare Advantage members with heart failure. Over five years of claims data, enrolled members were admitted less often, readmitted less often, and cost the plan 11% less. Published in Population Health Management.
